NEW PATIENTS

  • New patients using insurance must bring the following
    • Referral from a physician (MUST HAVE IT FOR YOUR FIRST APPT.)
    • Insurance Card(s)
    • VALID ID (Driver's license, State ID, Military ID, Passport etc.)
    • Op-Note (Surgery Note) (if applicable)
    • X-Rays, MRI & CT Scans (if applicable)
    • Primary care physician (PCP) referral, if required by your insurance
  • New patients using workers' compensation insurance must bring the following
    • Referral from a physician (MUST HAVE IT FOR YOUR FIRST APPT.)
    • Claim Number
    • VALID ID (Driver's license, State ID, Military ID, Passport etc.)
    • Op-Note (Surgery Note) (if applicable)
    • X-Rays, MRI & CT Scans (if applicable)
  • It is the patient's responsibility to confirm that we participate with his/her insurance plan and also to bring a Primary care physician (PCP) referral, if required by your insurance.
  • Please arrive 20 to 25 minutes before your initial appointment to allow time for paperwork.
  • You may arrive on time for your initial appointment if you bring the completely filled and printed forms.

CANCELLATION POLICY

  • Missed Appointments/Cancellations/Rescheduling requests that occur less than 24 hours before the scheduled appointment are subject to a $80 fee in all circumstances, including illness.  Late arrival will shorten your treatment time. Cancellation/rescheduling requests must be submitted via e-mail or voicemail if you are unable to reach us by phone.  Arrival fifteen minutes or more after your treatment time will be considered as a “no show” and the cancellation fee will apply.

 

INCLEMENT WEATHER POLICY

  • During the winter months, DC Hand Therapy is willing to make the following exception to the cancellation policy.  If the DC metro area is under a winter weather advisory and you believe that it would be unsafe or impossible for you to travel to your appointment, you may reschedule within the 24-hour window at no charge. Rescheduling requests must be submitted via e-mail or voicemail if you are unable to reach us by phone.
  • DC Hand Therapy follows the Federal Government's Office of Personnel Management (OPM) guidelines concerning dismissals and closures of offices in the event of emergency situations, adverse weather conditions, natural disasters, or other incidents causing disruption to operations.
  • The OPM Operating Status can be found online at: http://opm.gov/status/ 
  • If the Federal Government (in Washington, DC) is operating with a two hour delay, DC Hand Therapy will open at 10 a.m.  All appointments before 10 a.m. will be canceled.
  • If the Federal Government (in Washington, DC) is  closed, DC Hand Therapy is closed.
